With reference to the first two comments I have a white label 12" of this, but it is not single sided. It appears to have the same three tracks as the CD. The plain sleeve contains a plug sticker with the exact same wording as that mentioned by djemmay.

Why not submit it to Discogs then mate, you could copy draft of the CD and then made the relevant changes to signify it is a record.
