The front and back of the cover is a silkscreen brown grocery store bag. The rear image of a ship shows no words or an address like other versions. It contains one double sided foldout insert which identifies the band as "Heroin is still:". On the back of this insert, the P.O. Box number within the address differs from most of the other versions (see photo). Also contains a one-sided Gravity records catalog insert. Black and white labels with Side A: "GRAVITY 1." Side B: "2." The matrix on both side A and B are identical to the first press.
